A Multidisciplinary Approach to Managing Severe Gummy Smile Using 3D Simulation and Digital Surgical Guide: A Case Report

Abstract
A smile that reveals more than 3–4 mm of gum tissue is called a gummy smile (GS), which can negatively affect self-confidence and aesthetic appearance. Treatment planning for GS should consider underlying causes such as altered passive eruption, vertical maxillary excess, or hyperactive lip muscles. This case report describes a multidisciplinary approach for treating severe GS using digital planning and a surgical guide. The treatment combined orthodontic therapy with gingivoplasty supported by three-dimensional simulation and a digitally fabricated surgical guide. The digital workflow enabled precise diagnosis, improved surgical accuracy, and more predictable treatment outcomes. The results demonstrated significant aesthetic improvement and patient satisfaction, highlighting that digitally guided gingivoplasty can be an effective and minimally invasive alternative to more extensive surgical procedures for correcting severe gum exposure.
