Treatment planning for GS should consider underlying causes such as altered passive eruption, vertical maxillary excess, or hyperactive lip muscles. This case report describes a multidisciplinary approach for treating severe GS using digital planning and a surgical guide. The treatment combined orthodontic therapy with gingivoplasty supported by three-dimensional simulation and a digitally fabricated surgical guide. The digital workflow enabled precise diagnosis, improved surgical accuracy, and more predictable treatment outcomes.
